Job Description:

The Registered Nurse (RN) Case Manager is responsible for delivering comprehensive case management services to assigned Senior Care Options (SCO) members with complex healthcare needs. The RN ensures the delivery of high-quality, cost-effective care by developing, implementing, and evaluating individualized care plans. This role adheres to professional nursing standards.Key Responsibilities:

• Develop and regularly update individualized care plans based on physician orders, patient assessments, and input from the interdisciplinary care team.
• Complete the MassHealth Minimum Data Set (MDS) in accordance with organizational policy.
• Provide competent and quality care to patients in their homes.
• Manage a consistent caseload, ensuring each member receives ongoing attention and appropriate care.
• Oversee the execution and evaluation of each patient's care plan.
• Maintain effective communication with staff, and community providers as appropriate.
• Deliver patient and family education to promote understanding and self-care.
• Collaborate closely with both members and their families to support care goals.
• Participate actively in team meetings and knowledge-sharing initiatives.
• Perform additional duties as assigned.Qualifications:
  • Active Registered Nurse (RN) license in the Commonwealth of Massachusetts.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Minimum of 3 years of RN experience, ideally 5 years, including home care and clinical case management.
  • Proficiency in Microsoft Office and familiarity with case management software systems.
  • Ability to travel frequently within the local area.
