• Employment Type: TravelJob Description:Supplemental Health Care is seeking Registered Nurses in East Weymouth, Massachusetts with prior OASIS/Medicare Certified Visits for contract assignments with a partnering home health agency.
  
  Home Health RN Contract Details:- $2,900 - $3,100 per week- SHC Home Health Nurses are paid by the hour, not by the visit- Travel and mileage reimbursement available on all contracts- Nurse will have a designated territory of coverage - Weekly expectation is 30 units of productivity - Home Health contracts with SHC require previous experience with OASIS/Medicare Certified Visits- Goal is to improve outcomes, quality of life and decrease the cost of care.- Identify member needs- Take practical actions for proactive care- Manage transitions to eliminate gaps in care- Heavy documentation- Will be completing all visits in the patient's home (or other specific location agreed upon)- Heavy patient education- Helping to manage comorbid conditions (hypertension, diabetes, etc.) with diet, nutrition, exercise, and
  
  medication compliance.- Will be supporting members by taking basic vitals (weight, BP, HR)- Must be a Road Warrior- Travel within 60 mile Radius of territory- No on-call for overnight/weekends- Must be flexible with scheduling.- 3-5 patients per day- Member preference always comes first - Ex: if patient needs to schedule after 5pm or on a weekend to be seen, MH will
  
  accommodate. These are "one-off" scenarios. Typically, appointments will be scheduled M-F 8:30
  
  am through 4:30 pm- No hard "end time" for working day- EMRs (Electronic Medical Record): Salesforce is the primary documentation tool- Mileage/travel reimbursement: Must use Concur GPS app and own vehicle- Reimbursed at $0.44/mile- Use personal phone - must have unlimited data/hotspot- Reimbursed $65/month for cell phone (first check of the month)- Ring Central - app with business line assigned- Expect patient calls from 8 am-10 pm in your time zone
