Job SummaryWe are seeking a highly skilled Clinical Social Worker to join our team at the Boston Children's Hospital Primary Care Center in Weymouth. As a Clinical Social Worker, you will play a vital role in providing clinical social work services to patients and families, conducting psychosocial and risk assessments, and developing and implementing interventions and treatment plans.Key Responsibilities

• Provide clinical social work services to assist patients and families with personal, family, medical, and environmental difficulties.
• Conduct psychosocial and risk assessments, assign diagnoses, and develop and implement interventions and treatment plans.
• Participate in planning and implementation of discharge and outpatient service plans and collaborate with the healthcare team and community agencies to plan and implement treatment and provision of needed services.Requirements
  • Master's Degree in Social Work and LICSW is required.
  • Flexibility and ability to work in a fast-paced multidisciplinary team environment, as well as excellent communication skills.
  • Experience with social services agencies, understanding of social determinants of health, and trauma ideal.
  • Bilingual/bicultural/diverse candidates are encouraged to apply. Spanish speaking a plus.What We Offer
