Job Description:

30-hour Social Worker needed for Boston Children's Hospital Primary Care Center in Weymouth.
This role provides clinical social work services to patients and families with personal, family, medical and environmental difficulties.
Responsibilities include:

• 
  
• Providing psychosocial and risk assessments, assigning diagnoses and developing and implementing interventions and treatment plans.
  
• Participating in planning and implementation of discharge and outpatient service plans and collaborating with healthcare team and community agencies.
  
  To qualify, you must have:
  
  • 
    
  • Master's Degree in Social Work and LICSW is required.
    
  • Flexibility and ability to work in fast-paced multidisciplinary team environment as well as excellent communication skills.
    
  • Experience with social services agencies, understanding of social determinants of health and trauma ideal.
