  • Pet insuranceWhat You'll Do:As a Behavioral Health RN Case Manager, you'll take the lead in:Assessing patient needs-physical, psychosocial, and environmental-to develop individualized care plansManaging a caseload, coordinating, and evaluating treatment plansConducting initial evaluations and ongoing assessments during patient visitsCommunicating changes in patient conditions to the Clinical Supervisor, physicians, and care teamsEducating patients, caregivers, and staff on behavioral health philosophy and symptom managementProviding leadership and oversight to Behavioral Health Aides and LPNsOffering compassionate support to patients and familiesWhat You Need To Succeed:Current, unrestricted RN license in the state1+ years of nursing experience in a clinical care settingWillingness to travel within the assigned coverage area (50%)Reliable transportation, valid driver's license, & auto insuranceAssociate Or Bachelor's Degree In Nursing (preferred)Experience in Home Health, Hospice, or Homecare HomeBase (preferred)If you're ready to bring your passion for behavioral health nursing to a team that truly cares, apply today and be part of something meaningful at Elara Caring!As a growing organization, we invite you to share your information with us for consideration for future career opportunities.
