Job Description:

Lebanon Center Rehabilitation & Healing is seeking a dedicated and detail-oriented Licensed Practical Nurse (LPN) MDS (Minimum Data Set) Coordinator to join our team. As an LPN MDS Coordinator, you will be responsible for overseeing the assessment and analytical processes within our facility, ensuring both compliance with federal and state regulations and alignment with our center's high standards of care. Coordinate, conduct, and complete the Minimum Data Set (MDS), Care Area Assessments (CAA) and Care Plans in accordance with current federal and state regulations. Assist in the training of new nursing staff on MDS procedures. Collaborate with the healthcare team to gather patient information and track outcomes to ensure quality care and compliance. Analyze patient data to help in developing appropriate care plans and interventions. Ensure the accuracy and timeliness of all MDS assessments. Coordinate with healthcare team members to ensure comprehensive patient assessments. Serve as a liaison between nursing staff and administration to relay crucial information regarding patient care and treatment plans. Strong analytical and problem-solving skills Detail-oriented with high levels of accuracy in data handling Excellent interpersonal and communication skills Ability to manage time effectively and meet deadlines Knowledge of MDS 3.0 and RAI process Proficiency in clinical documentation and compliance management Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid LPN license in the state of employment Certification in MDS 3.0 preferred
